Sutter Pharmacy will be hosting vaccination clinics for the general public who are:

· 65 years and older, who lives in Yuba-Sutter counties
· 50-64 years old in education/childcare, food/agriculture, and emergency services who works in Yuba-Sutter counties
· Phase 1A providers - direct healthcare workers and residents of long-term care settings in Yuba-Sutter counties.

The pharmacy will be administering the Moderna COVID Vaccine, which is a 2-dose series.

This clinic is APPOINTMENT ONLY; no walk-ins can be accommodated. Appointments are extremely limited.
This clinic is for FIRST DOSES ONLY; do not register to receive your second dose for this clinic.

When you Arrive at the Clinic:
· Wear loose clothing that easily allows you to expose your arm to receive your shot.
· A mask will be required at all times during the clinic.
· Provide documentation for proof of eligibility.

Documentation Required for Proof of Eligibility:
· For those 65 and older: Driver's License, Utility Bill, passport, etc.
· For those 50-64 years old in education/childcare, food/agriculture, emergency services: Employer verification letter or paystub AND ID (driver's license, passport, etc.) for age verification
· Phase 1A Providers: Employer verification letter, IHSS paystub, etc.

Other Considerations
·If you received any vaccine within the last 14 days, please wait to receive the COVID-19 vaccination until the 14-day timeframe has passed.
·For those with insurances, please complete all sections (Insurance Name, ID, Group Number, Policy Holder Name and DOB, relation to client, etc.) to expediate the registration process on the day of the appointment.
·If you cannot upload an image of your insurance card, please upload a blank image to continue your online registration. Please present your insurance card on the day of your appointment.
·If there are no more appointments available, consider signing up to be wait-listed in the event the clinic has “no shows” or cancellations.
·The waitlist DOES NOT GUARANTEE you a spot for an appointment for the clinic.
·In the event you do not get called, please register for the next clinic.
