Forms

Each Alternative/Advanced OWTS must have an annual permit to operate. There are several forms that accompany this type of permit and they must be completed prior to issuance and/or final of the OWTS construction permit.

The Application form must be completed by the owner of the OWTS and it includes a space for indicating your contract maintenance provider.

The Acknolwedgement Document must be completed by the owner. This document notifies the owner of the OWTS that this system is being allowed for installation due to the adverse conditions on the parcel that may affect sewage disposal. It also acknowledges the need for the yearly operations permit through YCEH.

The Right of Entry Agreement must be completed by the owner and recorded with the Yuba County Recorder's Office prior to issuance of the permit to construct the alternative advanced OWTS. This document allows inspectors from YCEH access to the property for inspection of the OWTS for the purpose of observing, testing, sampling, placing and removing of test devices and samples, evaluating, monitoring and determining conformance with the Yuba County Sewage Ordinance.
