A Public Water Program Domestic Water Supply permit is required in order to operate a new Public Water System. The application form along with the appropriate Bacteriological Sample Siting Plan (BSSP), Emergency Notification Plan and TMF Assessment must be completed and submitted to the LPA prior to operation.

A Domestic Water Supply permit amendment is required if changes or modifications are going to be made to the existing permitted public water system. Changes and modifications include, but are not limited to the following:
  • Adding a new storage tank
  • Adding a new source
  • Changing the status of an existing source (e.g., from standby to active)
  • Adding or changing the treatment plant
  • Changes in classification of the Public Water System
