Documentation Required:

Ages 18 and older:
--Driver's license, passport, etc. to verify person receiving the dose is at least 18 years of age (if person wanting to get vaccinated is under age 18, written consent or accompaniment of a parent is required).


Ages 5-17 (Pfizer vaccine only):
--If minor is accompanied by a parent/guardian, no documentation is required.
--If the parent/guardian cannot accompany the minor, a signed written consent is acceptable (found in English HERE) (En Español HERE), (in Hmong HERE).
--The written consent must verify the parent/guardian has been provided the Pfizer EUA Fact Sheet.

Ages 6 months and older:
CDC recommends the COVID-19 vaccine for young children starting at age 6 months. The approved vaccines for this age group include Pfizer and Moderna. Talk to your child's physician about where to get vaccinated.

When you arrive to the clinic:
--Please eat before arriving to the clinic.
--Please arrive no more than 15 minutes before your appointment time.
--Wear loose clothing that easily allows you the expose your arm to receive your shot.
--A mask will be required at all times during the clinic.
